Change of holder data – must be announced in writing

 

  • To announce a change of holder data, please use the blue form for change of holder data (formerly pink or white).
  • Please send the filled in form to our office.
  • In case there is no such form for change of holder data available, you may send us a copy of the vaccination card or a copy of the pet passport, including the following information:
  1.  Number of microchip
  2.  Data of the animal
  3.  Name and first name of the former holder
  4.  Name and first name, address and phone numbers of the new holder
